 Form+Function is a movement lab, a space to help you solve painful problems, improve athletic performance, and own your movement practice. We have found that helping clients move better through the exploration of movement and a hands-on approach is the most effective way to improve pain and achieve athletic potential. We spend time empowering our clients through movement education to maximize their quality of life and reach performance goals.

Dr. Erik Nabi

Doctor of Chiropractic

Dr. Erik Nabi was born and raised in Buffalo, NY (go Bills!). He grew up with one older brother, Tarik, and a loving, supportive family. Erik found an interest in healthcare from an early age. His Father is a medical doctor and began taking him to observe his surgeries and patient rounds when Erik turned 12 years old. Dr. Nabi has cultivated a lifelong passion for fitness and athletic development. He played baseball and threw javelin at the division one level for the University of North Carolina at Asheville. Following his collegiate career, he retired to become a gym rat and fitness enthusiast, picking up weightlifting, road biking, playing various recreational sports, and teaching fitness classes in his free time. Throughout his athletic endeavors, Dr. Nabi suffered from multiple athletic injuries and developed an interest in chiropractic and function medicine. In 2015, he graduated with his Doctor of Chiropractic degree from Palmer College of Chiropractic. He has since been on an endless pursuit of honing

Nate Grisby

Functional Movement Coach

Coach Nate was born and raised in Toledo, OH, where he competed in a variety of sports including baseball, football, and golf. Throughout his athletic career, Coach Nate experienced multiple sports-related injuries that threatened his ability to stay on the field. In his efforts to resolve these injuries, he sought the help of medical practitioners and movement coaches, which allowed him to continue training and competing throughout his high school career. His experiences working with these individuals sparked his interest to help others overcome their injuries and strengthen their bodies. In 2015, Nate graduated from the University of Cincinnati with a bachelor’s degree in Sports Dietetics and Exercise Physiology. He began developing his functional training skill set through internships at local sports injury clinics. Here, he worked closely with chiropractors and physical therapists that introduced him to Dynamic Neuromuscular Stabilization (DNS), and further increased his passion for helping individuals solve movement-related problems. He subsequently developed a unique approach to training that combines a skill set in clinical therapy movement education with fitness performance programming. When Nate is not training clients, he loves spending the majority of his free time with his wife Jessica and their two children, Nora & Gavin. He keeps an active lifestyle by weight training, playing golf, hiking with his dog Rhett, and snow skiing.

Dr. Taylor Swint

Doctor of Physical Therapy

Dr. Taylor Swint, a Toledo, OH native, developed a passion for competitive sports early on, starting as a gymnast before excelling in volleyball and basketball at St. Ursula Academy. She earned a bachelor’s degree in health sciences from The Ohio State University, where she discovered long-distance running and completed her first marathon. Driven by her love for fitness and a desire to help others, she pursued a Doctorate in Physical Therapy at the University of Cincinnati and became a Certified Strength and Conditioning Specialist (CSCS) to bridge the gap between rehab and athletic performance. Initially focused on sports and orthopedic injuries, she later specialized in pelvic health after recognizing a lack of comprehensive care for female athletes. She is passionate about helping women improve pelvic health, particularly during pregnancy, postpartum, and in fitness-related activities. She integrates Dynamic Neuromuscular Stabilization (DNS) with hands-on techniques such as dry needling, cupping, joint manipulation, and soft tissue therapy to optimize recovery and performance. Outside of work, she enjoys time with her husband, Kyle, and their dog, Jovie, while staying active through powerlifting, running, and CrossFit.

Claire Davison

Functional Movement Coach

Claire was born and educated in England. She received an honors degree in Chemistry from the University of Bristol and went on to have a long career with Procter & Gamble. After almost 28 years with P&G, working on two continents and in three cities (London, Baltimore, Cincinnati), she decided to change direction in her career. In 2022, she studied with the American Council for Exercise and certified as a Personal Trainer. When Claire left England, she was no longer able to compete in her favorite team sport, Netball, which she had played in school, university, and then in a competitive club league. In the US, she took up long-distance running, taking part in several half marathons and one full marathon. In more recent years, Claire started working out and lifting weights in the gym, finding a passion for understanding how the body moves and works. Outside the gym, Claire likes to stay active and be outdoors. She loves to walk, run, birdwatch, hike, backpack, kayak, and cycle. She lives in Columbia Tusculum with her partner, Chris, and enjoys everything that the neighborhood has to offer.
